Services: Second Harvest’s core mission is to bring community resources together to feed people in need through empowerment, education and partnerships. Since 1971, Second Harvest has been the hub for charitable food distribution in the Inland Northwest. Right now, Second Harvest secures and distributes donated food to a network of 100 partner food banks, meal sites and other hunger-relief programs in Spokane County. (In addition, 150 other partner agencies are located in 20 other Eastern Washington Counties and five counties in North Idaho.) Second Harvest serves primarily Medicaid-eligible clients. Almost half of the food Second Harvest provides is nutrient-rich fresh produce. Through a teaching kitchen and broader nutrition education outreach, Second Harvest helps people move from hunger to health and self-sufficiency.

Second Harvest could play a role in the Collaborative-care model in the following ways:

Partnering with primary care providers to increase healthy food access to prevent rehospitalization and emergency care needs, and promote chronic disease prevention and management (e.g., bring Mobile Market to clinic setting, assist with developing healthy food pantry in clinic setting, develop produce prescription model for Medicaid patients);

Provide nutrition education designed to build healthier eating habits;

Coordinate Mobile Market free food distributions (featuring healthy fresh produce) in settings accessible by Medicaid patients that also feature other community resources.
