Successful Bidders for Pathways Hub RFP

BHT, with the help of three partnering providers, reviewed submissions from six Care Coordinating Agency (CCA) applicants who responded to our Pathways Hub Request for Proposals. While each organization has strong care coordination programs, we selected the two who we felt could immediately serve the criminal justice-involved clients of this Pathways Hub Pilot launching November 2018.

We are pleased to announce that we will offer contracts to Community-Minded Enterprises and Spokane Neighborhood Action Partners to provide direct care coordination to at least 200 clients through September 2020.

PROJECT TITLE: Pathways Community Hub Program Evaluation

PROPOSAL DUE DATE: July 20, 2018 by 5:00 p.m. Pacific Time, Spokane, Washington, USA.

E-mailed bids will be accepted at transformationprojects@betterhealthtogether.org. Faxed bids will not.

ESTIMATED TIME PERIOD FOR CONTRACT: August 1, 2018 to August 1, 2019
Better Health Together reserves the right to extend the contract for up to four (4) additional one (1) year periods at the sole discretion of the Better Health Together Accountable Community of Health.

BIDDER ELIGIBILITY: This procurement is open to those Bidders that satisfy the minimum qualifications stated herein and that are available for work in Washington State.
